Google Forms for MVP learning material and veterans/families applying to receive commemorative music:

Information you voluntarily provide via Google Forms is used to be included in Music for Veterans Project material. This information will be stored for an indefinite period of time. If you wish to have information you submitted on these forms be permanently removed, please use the contact page and we can delete your Google Form responses. Information submitted through the forms will only be used so that you can be contacted regarding MVP and the information you provide is that which you personally decide to share. The content you provide on any of the MVP forms will be used to create a printable learning module for a student composer assigned to compose your commemorative piece. Please contact us directly for an example of this featuring Pte. Bloggins. The photo provided will be included in the final video montage for the given year, or commemorative project.
